To help expedite the admission process, our staff will take some information over the phone such as demographics, health insurance information, and method of payment. We will verify insurance benefits prior to your initial visit to the clinic. In order to serve you better, we have included a link on this page to the admission documents. After completing them in the comfort of your own home you can bring them in at the time of your first visit. We do file directly to your insurance carrier, however the patient is responsible for their co-payment, deductible, or non-covered amounts at the time of service. If there is a question of coverage, we will contact you to resolve any payment issues.<br><br>We ask that you bring your referral or prescription with you on your first visit.<br><br>In order to serve you better, we have included a link on this page to the New Patient Packet and Patient Referral Form.<br><br>Q: Who will carry out the plan of care?<br><br>A: Physical Therapist - Educational requirements of at least a Bachelors Degree (sometimes a Masters or Doctorate) in Physical Therapy is required. The therapist is licensed by the state to evaluate and treat patients with a physician’s order. They also serve as consultants with other health professionals such as physicians, nurses, social workers, dietitians, and occupational therapists.<br><br>Physical Therapy Assistant - Educational requirement of an Associates Degree in Physical Therapy is required. The assistant is licensed to carry out the plan of care designed by your therapist.<br><br>Physical Therapy Technician - Technicians provide treatment under the supervision of the physical therapist or physical therapy assistant.<br><br>Q: What can I expect during my treatments?<br><br>A: During your first visit, one of our specially trained physical therapists will examine your ability to function in areas such as movement, strength, and range of motion.<br><br>Next, you and your therapist will create a treatment plan to accomplish your specific rehabilitation needs.<br><br>Each treatment can last up to one hour and it is important to wear comfortable and loose-fitting clothing.<br><br>We are pleased that you have chose P.T.S for your care and will do everything possible to ensure that you have a positive experience while in your rehabilitation.<br><br><hr/>
